Activating your kp.org account
Q. How do I activate my kp.org account?
A. Registering and activating your kp.org account is easy. Visit kp.org/register. Click “Register on kp.org” then follow the simple steps provided.
You will be asked to provide your member information, agree to terms and conditions, select any Kaiser Permanente e-mails that you would like to receive, select a user ID, and confirm your mailing address.
To keep your personal information secure, we will ask you to enter the last four digits of your social security number and answer a series of security questions. You will then be able to create your kp.org password and activate your account online.
If you prefer, we can mail you a temporary password. When you receive your temporary password (usually within three to seven business days), you can sign on to kp.org, create a new password, and activate your account.
Q. How will I know that my personal information is secure and kept private?
A. The information you provide to us online will be translated into a secret code before it is transferred to kp.org. That means no one can intercept your information. We use Secure Sockets Layer (SSL) to encrypt personal information. SSL is a standard communications method that's built into most commercial browsers to keep sensitive data secure as it's transmitted over the Internet.

Total health assessment
Q. What is the total health assessment?
A. The total health assessment or HealthMedia™ Succeed® is a questionnaire designed to help you take inventory of your health. It can help you understand how your behaviors affect your health by asking you a series of questions about yourself and the way you live. Once you answer all of the questions on the questionnaire, your responses will help us create a personal health improvement plan just for you!
Q. How do I access the total health assessment?
A. The total health assessment is only available online to members who have activated their kp.org accounts. To access your total health assessment, follow these steps:
- Visit kp.org/healthylifestyles.
- Click “Get a personalized total health assessment.”
- Sign on using your kp.org user ID and password.
- Click “Continue on to healthy lifestyles programs.”
- Follow the prompts to the HealthMedia, Inc. Web site.
- Choose HealthMedia™ Succeed® from the menu.
- Click “Start your questionnaire.”
- Before you submit your questionnaire, you will be asked for permission to allow HealthMedia to provide information to Kaiser Permanente for the drawing. This information only includes your name, address, date of birth, and other information necessary for issuing a reward.

Q. What are the healthy lifestyles programs?
A. Healthy lifestyles programs are personalized programs brought to you by HealthMedia, Inc. and Kaiser Permenente to help you achieve your own personal health goals. The programs include:
- HealthMedia™ Succeed® for your total health assessment.
- HealthMedia™ Balance® to help you maintain a healthy weight.
- HealthMedia™ Nourish® to help you make smart food choices.
- HealthMedia™ Breathe® to help you quit smoking.
- HealthMedia™ Relax® to help you deal with stress.
- HealthMedia™ Care® for Your Health to help with chronic conditions.
- HealthMedia™ Care® for Pain to help with chronic pain.

Q. Must I complete the total health assessment through HealthMedia™Succeed® before I complete another healthy lifestyles program?
A. No, but it might be helpful because it can help you choose which program or programs may be best suited for you to try. Also, it can help direct you to various services available through the healthy lifestyles programs.
Q. Is there a limit on how many healthy lifestyles programs I can take?
A. Members have access to all of the free healthy lifestyles programs. In order to become eligible for the quarterly UC drawings however, you must complete the healthy assessment program, HealthMedia™Succeed ®.

Quarterly drawings for UC employees
Q. What is the UC health assessment rewards program?
A. UC employees who complete the Kaiser Permanente HealthMedia™Succeed® (Health Assessment) will be eligible to win prizes in quarterly drawings. Prizes include one $500 gift certificate for spafinder.com, REI®, Big 5®, or Sports Authority® and 6 80GB color-screen iPods®.
Q. What if I complete the Kaiser Permanente health assessment questionnaire before I knew about the opportunity to participate in a quarterly drawing for prizes?
A.  If you completed the Kaiser Permanente health assessment after January 1, 2008, you’ll automatically be entered in the first drawing. If you completed the questionnaire before this date, you’ll need to wait until 12 months after you first submitted your health assessment. You’ll then be eligible for the next quarterly drawing.
Q. How will I know if I am eligible for the drawing?
A. You will be contacted by HealthMedia if you submit your questionnaire AND open your personal health summary in time to be eligible for the next quarterly drawing.
Q. When will the drawings be held?
A. The first quarterly drawing will be on July 18. Subsequent drawings will be October 22, and January 31, 2009. The cut-off date for the October drawing is September 30; for the January drawing, it's January 15.
Q. Are there any tax implications to receiving these prizes?
A. Rewards may be taxable. Kaiser Permanente members who earn rewards are responsible for any applicable taxes due on the amounts received. For additional information regarding any potential taxes associated with these prizes, please consult your tax professional.
Q. Are Medicare members, both Kaiser Permanente Senior Advantage and Cost members, eligible for the rewards prgrams?
A. No. Medicare marketing rules prohibit health plans from giving "cash or cash equivalents" to Medicare members, including Kaiser Permanente Senior Advantage and Medicare Cost members. This rule applies regardless of whether Medicare coverage is primary or secondary.
Q. Will the University of California know if I have activated a kp.org account?
A. No. The University of California will NOT know you have activated a kp.org account.
Q. Will the University of California know if I have taken a health assessment or have participated in any of the healthy lifestyles programs?
A. No. The University of California will not know if you take a health assessment or have participated in any of the Healthy Lifestyles Programs.
Q. Will Kaiser Permanente know if I have taken a health assessment or have participated in any of the online healthy lifestyles programs?
A. Kaiser Permanente will only know if you have won the quarterly drawing so they can administer your prize. The drawing itself is conducted by Kaiser’s partner, HealthMedia, who will contact Kaiser with the winner’s contact information.
However, upon completion of the health assessment, you will be given the option to download your results directly to your electronic medical record. By doing so, you will be sharing your information with your Kaiser Permanente health care providers. Anyone who takes the health assessment will have this option.
The feature of the electronic medical record that allows providers to view a patient’s health assessment summary will be fully implemented in Northern California in August of 2008. If you take the health assessment after January 1, 2008, your summary will become available to your doctor in August. The feature is already fully functional in Southern California.
Q. Why would I share the results of my health assessment with my Kaiser Permanente health care provider(s)?
A. When you complete your health assessment, you will be asked if you want the results downloaded to your electronic medical record so your Kaiser Permanente health care providers may view it. Your results can help foster discussions with your provider regarding your health and any lifestyle changes you’re thinking of making.

Q. Who should I contact if I have further questions?
A. You may contact Kaiser Permanente Member Services at 1-800-464-4000 between 7 a.m. and 7 p.m. PST Monday through Friday, and 7 a.m. and 3 p.m. PST Saturday and Sunday. Be sure to have your Kaiser Medical Record number available. This can be found on the front of your ID card.
