Completion of the Kaiser Permanente enrollment form must also accompany a completed University of Akron Personal Enrollment Form (PEF) for any reason—new hire, qualified family status change, or annual open enrollment.
New hires and employees with qualified family status changes must contact Benefits Administration at (330) 972-7092 to initiate the PEF form.
Annual open enrollment PEF forms may be completed online by following these instructions:
| 1. | Go to ZipLine and log in |
| 2. | Click on the Faculty/Staff tab, if you are not there already |
| 3. | In the Employee Services box, select Benefits |
| 4. | Choose Benefits Enrollment |
