San Francisco Health Service System offers infertility benefits

Whether you are in a same-sex relationship, a single man or woman, when it comes to your fertility needs, you’re in good hands. Your doctor will help you understand your options and decide what kind of treatment is right for you.

Covered Services

The list below is a high-level overview of the Assisted Reproductive Technology (ART) procedures that are available for members, including spouses, domestic partners, and children up to 26 years of age. For a complete list of services and limitations, see your plan’s Evidence of Coverage.

  • Physician consultation
  • Services related to conception by artificial means
    • In vitro fertilization (IVF)
    • Gamete intrafallopian transfer (GIFT)
    • Zygote intrafallopian transfer (ZIFT)
  • Inpatient hospital services
  • Inpatient physician services
  • Artificial insemination
  • Donor sperm insemination
  • Genetic testing
  • Male fertility procedures
  • Ultrasounds
  • Fertility drugs

The fertility benefit under the City and County of San Francisco’s HMO plan is covered at a 50% coinsurance. This member cost share is not subject to the plan Out-of-Pocket-Maximum. In addition, members are covered for two treatment cycles per lifetime. For additional information about your fertility benefit, please see the plan’s Evidence of Coverage document.

LGBTQ Families

At the Kaiser Permanente Centers for Reproductive Health, we are dedicated to supporting all our patients in their journey to grow their family. We’re here to help you along the way with third-party fertility solutions at our clinics.

Treatment may require a sperm donor, an egg donor, and/or a gestational carrier. This treatment may also involve the transfer of eggs from one female partner to another. As applicable, one of our coordinators will work with you to conduct all the necessary donor and surrogacy screenings.

If you are a Kaiser Permanente member, the easiest way to transition to these services is to request a referral from your primary care provider or ob-gyn to one of our reproductive endocrinology and infertility (REI) providers or male infertility specialists.
