Wellness Program information

Getting started is as easy as 1, 2, 3!

Complete the 3 required healthy activities listed below and you will receive your wellness reward of $0 copays for all Primary Care Physician (PCP) visits. Complete these activities by the 15th of the month anytime in 2021 and the $0 PCP copay becomes effective the 1st of the following month. This benefit continues through December 31, 2022. The sooner in 2021 that you complete the activities, the sooner you earn your reward!

  1. Sign a Wellness Agreement
  2. Designate a Primary Care Physician (PCP) at Enrollment
  3. Complete Total Health Assessment

The State of Maryland wellness program applies to employed subscribers and their spouses and non-Medicare retirees and their spouses.

You may take the health assessment as often as you like, but you can only receive the reward for it, if available, once during the reward period.

Wellness programs and rewards are separate services that are not health plan benefits and may be discontinued at any time.

Sign a Wellness Agreement

In order to earn your wellness award, we need your permission to share with the State of Maryland that you have completed your activities. No personal health information is shared, only data related to activities completed.

Browse our doctor profiles to see education, credentials, specialties, profile photos, and everything else you need to know to select a doctor who’s right for you. And if you change your mind, you can switch doctors at any time, for any reason.

Complete Health Risk Assessment

As a Kaiser Permanente member, our Total Health Assessment (THA) will serve as your Health Risk Assessment (HRA). Answer questions about yourself and get a customized action plan. Once you’ve completed the assessment, it is important to discuss these results with your PCP.
