Annual Wellness Visit

At Kaiser Permanente, we want to help you stay healthy. We also want it to be easy for you to talk with your health care provider about maintaining a healthy lifestyle and keeping up with preventive screenings.

An Annual Wellness Visit at No Charge

Your OEBB coverage now includes an annual no charge wellness visit with your primary care provider.

During your yearly wellness visit, we will:

  • Assess your health risks by looking at your physical activity, nutrition, mood, and other factors.
  • Review your medical and family history.
  • Develop or update a list of your current providers and prescriptions.
  • Take basic measurements, including your height, weight, blood pressure, and other routine measurements.
  • Offer personalized health advice.
  • Share any risk factors and treatment options with you.

How to schedule your annual wellness visit

To schedule your optional annual wellness visit, please call 1-800-813-2000 toll free and press “0” to speak to an appointing representative. Ask for a “wellness visit” and let them know that you have OEBB coverage. The wellness visit is not a requirement of the Healthy Futures program.

How to prepare for your annual wellness visit

You will need to complete a Total Health Assessment (THA) questionnaire on and share the results with your primary care provider in order to be eligible for your annual wellness visit. This is so your clinician can review your health risks and prepare a personalized prevention plan specifically for your needs.

If you are participating in the Healthy Futures program, you may already have a current THA on file. If you are not sure whether your THA is current, you may retake the THA at any time. The wellness visit is available to all OEBB members, even if you are not participating in the Healthy Futures program, but you must complete the THA and share the results with your primary care provider.

Completing the THA

To complete the THA:

  • Go to– and log-in to your account to access the survey (you can also register for a new account if you have not done so in the past).
  • At the end of the survey you will be asked to select “YES” if you would like to add your THA results to your Kaiser Permanente medical record so your provider can review them. You must select this option to ensure the results go to your medical record and can be seen by your care team so they can prepare for your visit. Your individual THA results are 100% confidential and will not be shared with anyone but your clinician and care team.

You may receive a phone call from a member of your care team to discuss your THA results so that your clinician can develop a personalized prevention plan prior to your visit.

There is no charge for this visit. However, if you receive medically necessary care beyond what is described above, your share of the cost may apply.
