State of Maryland employees, Kaiser Permanente is focused on your health and wellness. Our members are at the center of everything we do. As encouragement, the State of Maryland wellness program offers you the opportunity to earn wellness rewards.
Complete the 3 required healthy activities and you will receive your wellness reward of $0 copays for all Primary Care Physician (PCP) visits:
- Sign a Wellness Agreement
- Designate a Primary Care Physician (PCP) at Enrollment
- Complete Total Health Assessment
Complete these activities by the 15th of the month and the $0 PCP copay becomes effective the 1st of the following month. This benefit continues through December 31, 2024. The sooner that you complete the activities, the sooner you earn your reward!
NOTE: The State of Maryland wellness program applies to employed subscribers and their spouses and non-Medicare retirees and their spouses.

You may take the health assessment as often as you like, but you can only receive the reward for it, if available, once during the reward period.
Wellness programs and rewards are separate services that are not health plan benefits and may be discontinued at any time.
