A. No. Your employer will not know you have activated a kp.org account unless you authorize this information to be shared in order to earn a reward. You can activate a kp.org account only if you are a Kaiser Permanente member.
A. Kaiser Permanente and your employer may know that you have taken a Total Health Assessment or another healthy lifestyle program only if you have authorized that information to be shared for purposes of administering your reward. The personal health information from your assessment will not be shared with Kaiser Permanente or your employer. If you choose, you may download your assessment results to your electronic medical record, which will make them available to your Kaiser Permanente health care providers.
A. When you complete your Total Health Assessment, you will be asked if you want the results downloaded to your electronic medical record so your Kaiser Permanente health care providers may view them. We offer you this option because we believe that your personal physician can provide better health care when he or she knows you better. You can use your results to help you discuss your health and any lifestyle changes you’re thinking of making with your physician.
Total Health Assessment
A. You cannot earn the Total Health Assessment reward retroactively.
If you completed the questionnaire before your rewards program became effective, you must take it again during your reward period. You can retake the Total Health Assessment as often as you would like, but you can only earn rewards, if available, once during the reward period.
Rewards
A. The reward administrator sends reward cards by standard first class mail within 4 to 6 weeks after completion of the wellness activity or at the end of the reward period. If merchandise rewards are available to you, the retailer will ship your selected merchandise to you directly.
A. No, the reward card is not a Kaiser Permanente identification card and cannot be used for identification purposes. To identify yourself to receive services and prescriptions at any Kaiser Permanente facility, you need to present your Kaiser Permanente identification card.
A. Expiration dates vary by card. Reward cards are subject to the terms and conditions (policies, redemptions, and expiration dates) established by the issuing retailer and are subject to change. Certain retailers or denominations may not be available.
A. Participants who earn rewards are responsible for any applicable taxes that may be due on the amounts received. Participants may find reward amounts reported either as employee compensation or miscellaneous income, depending on the payer. Reward program enrollees should consult their personal tax advisor for specific tax consequences and determination of tax liability.
A. Depending on the reward programs available to you, you can earn rewards as described below:
- You can earn a reward only once for registering at kp.org. (This reward is available to Kaiser Permanente members only.)
- You can retake the Total Health Assessment as often as you like, but you can only earn rewards, if applicable, once during the reward period.
- You can take each healthy lifestyle program, once every 201 days, but you can only earn rewards, if applicable, once during the reward period.
