How do I create secure accounts on the Sound Health and Kaiser Permanente websites?

It is important to register now with Kaiser Permanente’s member website to create a secure account. You’ll need your member ID number, which is on your Kaiser Permanente ID card.

To register on Sound Health & Wellness Trust’s website, go to www.soundhealthwellness.com and click on the “Register” button in the upper right portion of the screen. Fill in the information on the registration screens. At the end, you will be logged into your secure account.

Do I have to do these steps online?

The fastest way to complete these two steps is online. If you have an email address but do not have a computer at home, think about using a library or a friend’s computer. If you do not have an email address or are not able to use a computer:

  • Request that a paper Health Profile form be mailed to you by calling 1-866-458-5277. The paper version of the Health Profile is available only by calling this number. The deadline to call for the paper version is Sept. 16, 2024 so that you can receive it in the mail, complete it and return it in the postage-paid envelope postmarked no later than Sept. 30, 2024.
  • For assistance by telephone to select a primary care physician (PCP), call Kaiser Permanente customer service at 1-888-901-4636 by the Sept. 30, 2024 deadline.
Does my enrolled spouse have to complete these steps?

If you have a spouse enrolled in the plan, he or she must also complete these steps in order for your family to receive the maximum HRA funding and avoid a higher deductible. Your spouse will have received this letter as well. See Question 1 above if you or your spouse has not yet created a secure account.

What if I can't answer every question on the Health Profile?

Just complete what you can. Not all questions are required, but the more questions you answer, the more tailored and useful your results will be.

Why do I have to do these steps?

The Trust is continuing to encourage and reward healthy behaviors among our plan participants. Knowing your health status through answering the Health Profile questionnaire; keeping your contact information updated so that the Trust can let you know about benefits and programs available to you; and choosing a PCP who will be your health care partner for preventive care, medical treatment, and can help coordinate referrals to specialists, if needed, will give you many tools to take care of your health.

We also want to get as much participation as possible in our Sound Support programs. These programs are designed to give employees and their families direct access to the medical expertise, information, and personalized support they need to make better health decisions and enjoy healthier, happier lives. With healthier employees and families come lower medical costs, which allows us to continue offering excellent medical coverage.

What happens if I don't do these steps or any of the health-related actions?

If you don’t select or confirm your primary care physician, update your contact information, complete your Health Profile, or participate in any of the health-related actions, you will not get your full 2025 funding (up to $500 for employee-only coverage, or up to $1,000 for family coverage) and your 2025 deductible will be higher.

How soon after I complete my Health Profile will the additional credit show up in my HRA account?

Once you have completed the three steps to fund your 2025 HRA, your account will be credited with up to $300 in 2025 HRA funding in January 2025.

Who do I call if I have questions?

If you have questions about your HRA, call the Trust Office at 1-800-225-7620 and choose Option 2 then Option 1. If you have questions about the Kaiser Permanente website, call 1-888-901-4636.
